Alzheimer and Eldercare Services

If you are responsible for a person who is an Alzheimer or Eldercare patient who lives in, or frequently visits Bangor, you may be interested in this service.

The Bangor Police Department will put the patient name and other pertinent information into our dispatch database. If a patient should need the services of one of our police officers, we will have the information that you have provided to see that you are notified and that the patient receives immediate care. The information in our database is also available to all public safety dispatch centers in the greater Bangor area. These include Brewer, Hampden, Old Town, Orono, Penobscot County Sheriff's Department, University of Maine Department of Public Safety, and Veazie.

Please click on the application icon above to view and print the application. Please complete the form, have it notarized, and send it to the Bangor Police Department. A recent photograph of the patient is helpful, but not mandatory.

If you cannot download the form from the link above, please feel free to stop by the Bangor Police Department and follow the remainder of the instructions listed above.
