Occupational Health Services
The City of Bangor requests proposals from interested, qualified and experienced occupational medical facilities to perform City-wide occupational health services, specifically in the areas of pre-employment (post-offer) physical examinations, return to work, fitness for duty evaluations, annual physicals, drug and alcohol testing (DOT and Non-DOT), occupational medical surveillance, workers' compensation (evaluation, treatment, management, and referral services) and medical record management. The successful Proposer will serve as the City of Bangor's designated ten-day Occupational Healthcare Provider for the City's Worker's Compensation Program.
