   This is a survey for concerned citizens to voice their opinions. If you wish to file a formal complaint on an employee, you may start the process here. Make sure to check the box that indicates your interest in filing a formal complaint and you will receive the employee complaint form via email.

   If you prefer to pick a form up in person, you can do so at the police station. Ask for a form from the duty supervisor.
